This Consent provides information about telehealth and obtains your informed consent to receive care via telehealth from the independent, state-licensed providers who deliver services through the Oriah Wellness platform.

1. Acceptance of This Consent

This Telehealth Informed Consent (this "Consent") explains how healthcare services are delivered through telehealth by the independent, state-licensed physicians, physician assistants, and nurse practitioners (the "Providers") who treat patients using the platform operated by Oriah Wellness LLC ("Oriah," "we," "us," or "our") at oriahwellness.com (the "Service"), and obtains your informed consent to receive care in this manner. Through the Service, the Providers offer care across four areas only: physician-supervised weight-loss protocols (including compounded semaglutide and compounded tirzepatide), longevity and anti-aging peptide protocols (such as NAD+, sermorelin, ipamorelin, and BPC-157), hormone therapy and testosterone optimization (such as testosterone cypionate), and sexual-health treatments for erectile dysfunction (such as sildenafil and tadalafil). Care is personalized to your bloodwork and monitored by board-certified physicians, and any prescribed medications are dispensed by licensed U.S. pharmacy partners. The Service is offered to adults only, on a cash-pay basis; Oriah does not bill or accept insurance, Medicare, or Medicaid. In this Consent, "you" and "your" refer to the adult patient (age 18 or older) who creates an account and uses the Service to obtain care for themselves. Please read this Consent carefully before creating an account or using the Service. The "Last Updated" date of this Consent is February 4, 2026.

This Consent is Oriah's separate Telehealth Informed Consent, distinct from and supplementing the agreements referenced in our Terms & Conditions. It governs the clinical relationship between you and the independent Providers who furnish care through the Service. Oriah operates the technology platform and provides non-clinical administrative support only; Oriah does not practice medicine or provide medical advice, and the provider-patient relationship created through the Service is with the Providers, not with Oriah. Any laboratory testing referenced in your care may be performed by affiliated and/or third-party laboratories (such as national laboratories like Quest Diagnostics or Labcorp). Your acceptance of this Consent confirms your agreement to receive healthcare services from the Providers by means of telehealth as described in the sections that follow. 2. Emergencies and Crisis Support

Telehealth is not designed or intended for emergencies. The Service, including secure messaging with your care team, is not monitored in real time, and the independent, state-licensed physicians, physician assistants, and nurse practitioners who provide your care (your Providers) may not see or respond to your communications promptly. Oriah operates the technology platform and non-clinical administrative support only; Oriah does not practice medicine and does not provide emergency or urgent care. Do not use the Service to seek care for any urgent or emergent condition.

TELEHEALTH IS NOT FOR EMERGENCIES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM. The Service is appropriate only for non-emergency, non-urgent care relating to Oriah's offerings, which include physician-supervised weight-loss protocols, longevity and peptide protocols, hormone therapy, and sexual-health treatments. Please reserve secure messaging through the Service for routine, non-emergency matters. Symptoms that may require immediate, in-person evaluation include, but are not limited to, chest pain or pressure, difficulty breathing, severe or sudden allergic reactions, signs of a stroke, uncontrolled bleeding, fainting, or any other condition you reasonably believe to be serious or life-threatening. In any such case, do not wait for a response through the Service; call 911 or seek in-person emergency care immediately.

3. Purpose

The purpose of this Telehealth Informed Consent (this "Consent") is to provide you with information about telehealth and to obtain your informed consent to receive healthcare services through telehealth from the independent, state-licensed Providers who deliver care using the Oriah Wellness platform at oriahwellness.com (the "Service"). This Consent explains what telehealth is, how it is delivered through the Service, its anticipated benefits, its potential risks and limitations, and your related rights and responsibilities, so that you can make an informed and voluntary decision about whether to receive care in this manner.

The Service offers care in the following areas only: physician-supervised weight management using GLP-1 protocols (including compounded semaglutide and compounded tirzepatide); longevity and anti-aging peptide protocols (including NAD+, sermorelin, ipamorelin, and BPC-157); hormone therapy and testosterone optimization (including testosterone cypionate); and sexual health treatments for erectile dysfunction (including sildenafil and tadalafil). Care is personalized based on your intake information and bloodwork and is monitored by board-certified physicians. The Service is cash-pay only; Oriah does not accept or bill insurance, Medicare, or Medicaid.

As used in this Consent, "Providers" means the independent, state-licensed physicians, physician assistants, and nurse practitioners who evaluate you, make clinical determinations, and, where appropriate, prescribe treatment through the Service. The provider-patient relationship is between you and the Providers. Oriah Wellness LLC ("Oriah," "we," "us," or "our") operates the technology platform and provides non-clinical administrative support only; Oriah does not practice medicine, does not provide medical advice, and does not control, direct, or interfere with the independent clinical judgment of the Providers. Any medications prescribed through the Service are dispensed by licensed U.S. pharmacy partners, and any laboratory testing is performed by affiliated and/or third-party laboratories (such as national reference laboratories like Quest Diagnostics or Labcorp).

As used in this Consent, "you" and "your" refer to the adult patient who is at least 18 years of age and who is using the Service to obtain healthcare services for yourself. The Service is intended solely for adults seeking care on their own behalf, and you may not use the Service on behalf of any other person.

4. Use of Telehealth

Telehealth (also referred to as telemedicine) means the delivery of healthcare services and the exchange of medical information through electronic communications between you and an independent, state-licensed physician, physician assistant, or nurse practitioner (each, a Provider) who is located in a different place than you at the time of the interaction. As used in this Consent, "you" and "your" refer to the adult patient (18 years of age or older) who is using the Service for themselves.

Telehealth as delivered through the Service may include a range of electronic methods, such as interactive audio, live video, secure messaging, online intake questionnaires, and store-and-forward technology (the electronic transmission of your medical history, images, laboratory results, and other protected health information for review by a Provider at a later time). These communications may be synchronous (occurring in real time) or asynchronous (occurring at different times), and they may be used by your Provider for clinical evaluation, diagnosis, treatment, prescribing, ordering and reviewing laboratory testing, ongoing follow-up, and patient education.

Laboratory testing that supports your care may be performed by affiliated and/or third-party laboratories (which may include national reference laboratories such as Quest Diagnostics or Labcorp). Any medications prescribed by your Provider are dispensed and shipped to you by licensed U.S. pharmacy partners. The Service is offered on a cash-pay basis only; Oriah does not accept or bill insurance, Medicare, or Medicaid, and no claims are submitted to any third-party payor on your behalf.

Oriah Wellness LLC ("Oriah") operates the technology platform and provides non-clinical administrative support only. Oriah does not practice medicine, does not provide medical advice, and does not control the independent clinical judgment of any Provider. Your provider-patient relationship is with the Provider, not with Oriah. Through this telehealth model, Providers may evaluate, treat, and monitor patients across Oriah's four service lines:

  • Weight loss — physician-supervised GLP-1 protocols, including compounded semaglutide and compounded tirzepatide;
  • Longevity and anti-aging — peptide protocols, such as NAD+, sermorelin, ipamorelin, and BPC-157;
  • Hormone therapy — testosterone optimization and TRT, such as testosterone cypionate; and
  • Sexual health — ED treatments, such as sildenafil and tadalafil.

You understand that the Service is intended only for adults 18 years of age or older and is not appropriate for the treatment of minors. You further understand that telehealth has potential benefits — including improved access to care, greater convenience, and reduced travel — as well as potential limitations and risks. Because your Provider relies on information you supply and cannot conduct a hands-on physical examination, there is a possibility that a condition could go undetected, that information could be insufficient to make a clinical decision, or that technical problems (such as equipment failure, poor connection quality, or interruptions in electronic communication) could delay or disrupt your evaluation or treatment. Your Provider may determine that telehealth is not appropriate for some or all of your care and may recommend in-person evaluation or alternative treatment.

Telehealth is not the only way to receive care. Alternative methods of medical care, such as in-person services, may be available to you, and you may choose to seek an alternative method of care at any time. We encourage you to discuss the benefits, risks, and appropriateness of telehealth, as well as any available alternatives, with your Provider so that you can make an informed decision about your care. 5. Anticipated Benefits

Telehealth may offer you a number of potential benefits when you receive care from independent, state-licensed physicians, physician assistants, and nurse practitioners ("Providers") through the Service. Oriah Wellness LLC ("Oriah") operates the technology platform and provides non-clinical administrative support only; Oriah does not practice medicine, and your provider-patient relationship is with the Providers, not with Oriah. These benefits are possible outcomes only; they are not guaranteed, and the actual benefit to you depends on your individual circumstances, the nature of your condition, and the clinical judgment of your Provider. Anticipated benefits of receiving care through telehealth include the following:

  • Easier and more efficient access to care, without the need to travel to a physical office, across Oriah's service lines: weight loss (physician-supervised GLP-1 protocols), longevity and anti-aging (peptide protocols), hormone therapy (TRT / testosterone optimization), and sexual health (ED treatments).
  • The ability to obtain care at times that are convenient for you, rather than being limited to traditional in-office hours.
  • The opportunity to interact with your Provider and receive evaluation and treatment without scheduling or attending an in-office appointment.
  • More efficient and secure sharing of your medical history and bloodwork with your Provider to support evaluation of appropriate protocols. Laboratory testing, where indicated, may be performed through affiliated and/or third-party laboratories (for example, national labs such as Quest Diagnostics or Labcorp).
  • Ongoing monitoring of your care by board-certified Providers and timely adjustments to your protocol over the course of treatment.
  • Where your Provider determines that a prescription is appropriate, convenient fulfillment of medications, which are dispensed by licensed U.S. pharmacy partners and shipped to you.

The Service is intended solely for adults 18 years of age or older who are using the Service for themselves. Oriah operates on a cash-pay basis only; we do not accept or submit claims to insurance, Medicare, or Medicaid. The decision to pursue any treatment, and the specific protocol prescribed, rests with you and your Provider based on your individual evaluation. 6. Potential Risks

While telehealth offers meaningful benefits, you should understand that it also carries inherent risks and limitations. As with any form of medical care, the delivery of healthcare services through electronic communications cannot guarantee a particular result or outcome and may, in certain circumstances, be less effective than an in-person encounter. Oriah operates the technology platform and provides non-clinical administrative support only; Oriah does not itself practice medicine, and all clinical decisions, including diagnosis, treatment, and prescribing, are made by the independent, state-licensed physicians, physician assistants, and nurse practitioners (the "Providers") who deliver care through the Service. The Service supports a limited set of wellness offerings, namely physician-supervised weight-loss (GLP-1) protocols, longevity and peptide protocols, hormone therapy, and sexual-health treatments. By consenting to receive care through the Service, you acknowledge that you are at least 18 years of age, that you are using the Service for yourself, and that you have considered and accept the following potential risks. These risks include, but are not limited to, the following:

  • Limited clinical assessment. The quality, accuracy, and completeness of information transmitted electronically may be limited, and your Provider may not have access to the full range of diagnostic tools available during an in-person visit, which may affect the Provider's ability to evaluate your condition.
  • No hands-on examination. Your Provider will not perform a physical, hands-on examination and generally cannot obtain in-person vital signs or conduct certain in-person tests, which may affect the assessment of your suitability for treatment.
  • Reliance on laboratory results. Certain protocols are personalized to your bloodwork. Laboratory testing performed through affiliated and/or third-party laboratories (such as national labs like Quest Diagnostics or Labcorp) may be delayed, incomplete, or, in rare cases, inaccurate, which may affect your Provider's clinical decisions.
  • Medication-specific risks. The medications offered through the Service, including compounded GLP-1 medications, peptides, hormone therapies, and treatments for sexual health, carry their own risks, side effects, contraindications, and the potential for adverse reactions or drug interactions, which your Provider will discuss with you and which are addressed in any medication-specific consent.
  • Inability to treat your condition. Based on the information available through telehealth, your Provider may determine that they are unable to diagnose or treat your condition remotely, or that your condition is not appropriate for treatment via telehealth, in which case you may be advised to seek in-person care.
  • Technology failures. Electronic communication systems may contain bugs, errors, or defects; may be unavailable or interrupted; and may produce records or images that are incomplete, corrupted, or lost. Such failures could impair or delay diagnosis, treatment, prescribing, or follow-up.
  • Delays. Technical difficulties, system unavailability, or other factors may cause delays in evaluation, treatment, prescribing, shipment of medication from licensed U.S. pharmacy partners, or your Provider's response to your messages.
  • Security and privacy breaches. Despite reasonable safeguards, the security protocols protecting electronic transmissions could fail, or other circumstances could arise, that cause a breach of your privacy or of your protected health information.   • Susceptibility of electronic data. Information transmitted by email or other electronic means is more susceptible than other forms of communication to unintended disclosure of your protected health information.
  • Regulatory limits on prescribing. Federal or state laws and regulations may limit or prohibit the prescribing of certain medications via telehealth in some jurisdictions, which may restrict the treatment options available to you.
  • Cash-pay limitations. The Service is offered on a cash-pay basis only and does not bill insurance, Medicare, or Medicaid. Cost considerations are your responsibility, and an inability or unwillingness to continue paying for care or medication could interrupt your treatment.
  • Incomplete records. If your Provider does not have access to your complete medical history and records, there is an increased risk of adverse drug interactions, allergic reactions, or other errors in clinical judgment.

You can help reduce many of these risks by providing complete, accurate, and truthful information about your health history, current medications, allergies, and symptoms, and by promptly responding to your Provider's requests for additional information or laboratory testing. You should also maintain a relationship with a local in-person healthcare provider, as telehealth is not a substitute for in-person care in all situations. 7. Emergency Situations and Follow-Up Care

Telehealth is not appropriate for every medical situation and is not a substitute for in-person care in all cases. Certain conditions cannot be safely or effectively evaluated, diagnosed, or treated remotely, and your Provider may determine, in their independent professional judgment, that your needs are better addressed through an in-person examination, in-person laboratory testing, or a higher level of care. This is true across all of the Service's offerings, including weight-management (GLP-1) protocols, longevity and peptide protocols, hormone and testosterone therapy, and sexual-health treatment. Nothing in this Consent or your use of the Service is intended to discourage you from seeking in-person medical attention whenever your condition warrants it.

Communicating With Your Providers

For non-urgent and non-emergency matters, use the secure messaging features within the Service to communicate with your Providers. Because telehealth care through the Service is delivered both synchronously and asynchronously, your Providers may not respond to messages immediately, and you should not expect or rely upon a prompt response. Please observe the following:

  • Use secure messaging in the Service only for routine, non-emergency questions and follow-up regarding your care, including questions about your medications, dosing, side effects, or laboratory results.
  • Never use secure messaging, email, or any other feature of the Service to report a medical emergency or an urgent condition that requires immediate attention.

  • If your situation is urgent or emergent and you cannot reach your Providers, do not wait — call 911 or seek care at an emergency room or from an appropriately equipped in-person provider.

Maintaining Local and Follow-Up Care

You are responsible for establishing and maintaining a relationship with a local healthcare provider or primary care physician who can deliver in-person care, including physical examinations, hands-on testing, and emergency treatment that cannot be provided through telehealth. You agree to follow your Providers' instructions regarding follow-up care and monitoring (including any recommended laboratory testing performed through affiliated and/or third-party laboratories), to attend scheduled follow-up consultations, to share relevant information — including any laboratory results, medications, and supplements — with your local providers, and to promptly seek in-person care whenever your condition changes, worsens, or otherwise requires it.

Oriah operates the technology platform and non-clinical administrative support only; it does not practice medicine, does not establish a provider-patient relationship with you, and does not provide emergency, urgent, or in-person care. Your provider-patient relationship is with the independent, state-licensed Providers, and any prescribed medications are dispensed by licensed U.S. pharmacy partners. 8. Data Privacy and Protection

The electronic systems used to deliver telehealth through the Service incorporate security protocols designed to protect the privacy of your information and to safeguard your data against unauthorized access, alteration, or loss. Protected health information ("PHI") that personally identifies you will not be disclosed to third parties without your consent, except where disclosure is authorized or required by law or as otherwise described in your Provider's Notice of Privacy Practices or our Privacy Policy.

Oriah operates the technology platform and provides non-clinical, administrative support only; Oriah does not practice medicine, and your provider-patient relationship is with the independent, state-licensed physicians, physician assistants, and nurse practitioners ("Providers") who deliver your care. Information is shared only as needed to support your care across Oriah's weight loss, longevity, hormone therapy, and sexual health services.

Disclosures authorized or required by law include those made for the following purposes:

  • Consultation and treatment — sharing your information among your Providers, and with affiliated and/or third-party laboratories and licensed U.S. pharmacy partners, as needed to evaluate, prescribe, dispense, coordinate, and monitor your care;
  • Payment and billing — processing your payments and supporting related cash-pay billing and account functions; Oriah does not bill insurance, Medicare, or Medicaid;
  • Administrative purposes — supporting the non-clinical operation, security, and integrity of the Service; and
  • Mandatory legal reporting — disclosures required by applicable law, such as reporting a serious risk of danger to yourself or others, or reporting suspected abuse, where a Provider or Oriah is legally obligated to do so.

Your use of the Service may include email and other electronic communications that contain PHI. While Oriah maintains reasonable safeguards for the systems it controls, Oriah cannot guarantee the security or privacy of services you use to send or receive these communications, including your own email provider, personal devices, or network. You are responsible for using communication tools you reasonably believe to be secure and for protecting access to your own accounts and devices.

9. Laboratory Products and Services

Certain care provided through the Service includes laboratory testing. Because each of Oriah's protocols — including our weight loss (GLP-1), longevity/peptide, hormone therapy (TRT), and sexual health offerings — is personalized to your individual health profile, bloodwork and related laboratory results are an important part of how your Provider evaluates whether a given protocol is appropriate for you, establishes a clinical baseline, and monitors your response over time. You must be 18 years of age or older to use the Service and to undergo any laboratory testing arranged through it.

Oriah operates the technology platform and provides non-clinical administrative support only. Oriah does not perform laboratory analysis, does not practice medicine, and is not a laboratory. Laboratory testing is offered through the Service as a coordinated arrangement with affiliated and/or third-party laboratories (which may include national reference laboratories such as Quest Diagnostics or Labcorp). Your Provider — an independent, state-licensed physician, physician assistant, or nurse practitioner — is responsible for the clinical aspects of any testing, including determining whether testing is appropriate, reviewing and placing test requests, receiving and interpreting your results, and providing related counseling and follow-up. The laboratory that processes your specimen is responsible for performing the analysis.

How Specimens Are Collected

Depending on the test ordered and your location, your specimen may be collected by means of an at-home blood collection kit mailed to you for self-collection, or through an in-person blood draw at a participating collection site. Any blood draw or specimen collection involves certain risks, which may include discomfort or pain at the collection site, bruising, redness or swelling, bleeding, lightheadedness or fainting, and, rarely, infection. If you use an at-home collection kit, you are responsible for following the kit instructions carefully; improper collection, handling, labeling, or shipping may delay processing, require recollection, or affect the accuracy of your results.

Accuracy and Reliability of Results

Laboratory tests are third-party products, and neither Oriah nor your Provider guarantees their accuracy or reliability. No laboratory test is perfect. A test may produce a false negative, a false positive, or an inconclusive result, and a result that is accurate at the time of testing may not reflect your condition at a later time. Laboratory results are one input that your Provider considers as part of your overall clinical picture and are not, by themselves, a diagnosis. Self-collected and at-home specimens may, in some circumstances, be more susceptible to collection or handling variability than specimens collected by a trained professional.

Release of Results and Your Right of Access

Some states impose a waiting period before certain laboratory results may be released to a patient. By accepting this Consent, you elect to exercise your federal direct right of access to your laboratory results and knowingly and voluntarily waive any such state-imposed delay, so that your results may be made available to you and to your Provider as soon as they are completed.

Your laboratory results are confidential and are disclosed among the Service, the Providers, and the laboratory that processes your specimen for the purposes of providing and coordinating your care, consistent with applicable law and the privacy practices described in our Privacy Policy. You are responsible for sharing your results with your primary care physician or other local healthcare provider and for initiating any further evaluation, monitoring, or follow-up that your results may warrant.

Cost and Payment

The Service is cash-pay only. Oriah does not bill insurance, Medicare, or Medicaid, and does not submit claims on your behalf for laboratory testing arranged through the Service. The cost of any laboratory testing will be disclosed to you, and you are responsible for the applicable fees.

Important Limitations

  • The Service is not a laboratory and does not itself perform laboratory analysis.
  • You are not required to obtain laboratory testing through the Service. If you prefer, you may obtain comparable testing through another laboratory or local healthcare provider and share the results with your Provider for review.
  • The Service is not responsible for the retention, storage, or preservation of your specimen after it has been collected and submitted to the laboratory.
  • Laboratory results may be delayed, unavailable, or affected by collection or handling factors outside of Oriah's control.
  • Laboratory testing supports, but does not replace, the clinical judgment of your Provider and any in-person care recommended to you.
LABORATORY TESTING IS VOLUNTARY. YOU MAY DECLINE ANY RECOMMENDED TEST; HOWEVER, IF YOU DECLINE TESTING THAT YOUR PROVIDER DETERMINES IS NECESSARY, YOUR PROVIDER MAY BE UNABLE TO INITIATE OR CONTINUE A GIVEN PROTOCOL. LABORATORY TESTING IS NOT A SUBSTITUTE FOR EMERGENCY OR IN-PERSON CARE.

10. Open Payments Notice

This notice is provided for informational purposes only and does not alter the terms of your care or your relationship with the independent, state-licensed physicians, physician assistants, and nurse practitioners ("Providers") who treat you through the Service. Oriah operates the technology platform and non-clinical administrative support only; Oriah does not practice medicine.

The federal Physician Payments Sunshine Act (part of the Affordable Care Act) requires manufacturers of drugs, medical devices, and biologics to publicly report certain payments and other transfers of value worth more than ten dollars ($10) that they make to physicians and teaching hospitals. These reporting obligations apply to the manufacturers, not to Oriah, and Oriah is not a manufacturer of any drug, device, or biologic.

The federal Centers for Medicare & Medicaid Services (CMS) makes this reported information available to the public through its Open Payments database.   • You are at least 18 years of age and are using the Service for yourself as an adult patient.
  • Oriah operates the technology platform and provides non-clinical, administrative support only. Oriah does not practice medicine, and your provider-patient relationship is with your independent, state-licensed Provider and not with Oriah.
  • Healthcare services made available through the Service are provided to you by means of telehealth, including the electronic communication of your information between you and your Provider in different physical locations.
  • The Service supports a limited set of treatment areas, including weight management, longevity and anti-aging, hormone therapy, and sexual health. Your Provider will determine, in their sole professional judgment, whether any treatment is appropriate for you.
  • Your treating Provider may be a physician, a nurse practitioner, or a physician assistant, and not necessarily a physician. Where applicable, you agree to be evaluated and treated by such Providers, each of whom is independently licensed in your state.
  • The technology and electronic systems used to deliver telehealth may contain bugs, errors, or defects, may be unavailable or interrupted, and may otherwise affect the quality, accuracy, or effectiveness of the care you receive.
  • Laboratory and diagnostic services made available in connection with the Service, which may be performed by affiliated and/or third-party laboratories, may likewise contain defects or errors and may produce false, inconclusive, or inaccurate results.
  • Telehealth is an evolving field, and your care may make use of technologies or methods that are not specifically described in this Consent.
  • No particular benefits, outcomes, or specific results are guaranteed, including with respect to any laboratory result, diagnosis, or course of treatment.
  • Your condition may not improve, and in some cases may worsen, notwithstanding the care you receive through the Service.
  • Not every condition can be appropriately diagnosed or treated by means of telehealth, and your Provider may determine that your condition is not suitable for telehealth and may decline to provide treatment or refer you to in-person care.
  • There are potential risks associated with telehealth, including those described elsewhere in this Consent, and you accept those risks.
  • You have had the opportunity to discuss telehealth, including its anticipated benefits, potential risks, and available alternatives such as in-person care, with your Provider, and to ask questions and have them answered to your satisfaction.
  • There will be no recording of any treatment session, by you or by your Provider.
  • Your Provider evaluates your condition in their sole professional judgment and may determine that telehealth is or is not appropriate for you, and may refer you to in-person care or to a specialist. By continuing to use the Service, you concur with your Provider's assessment and agree to receive diagnosis and treatment by means of telehealth.
  • You may withdraw your consent to telehealth at any time by providing written notice to your Providers. Withdrawal is effective upon the Providers' receipt of your notice and does not affect any action taken in reliance on this Consent before it was withdrawn. You understand that the Providers do not offer in-person treatment.
  • Use of the Service involves the electronic communication of your protected health information (PHI), including by email, which may be less secure than other forms of communication. Information is handled as described in our Privacy Policy.
  • It is your duty to provide truthful, accurate, and complete information to your Providers, including current and accurate emergency contact information.
  • Oriah has a financial relationship with the professional entity that employs or contracts with your Provider, and with the licensed U.S. pharmacy partners that may dispense your medications.
  • You are free to obtain a medical examination or second opinion from another provider not associated with Oriah, and to have any prescription filled at any pharmacy of your choice by contacting support@oriahwellness.com. Prescriptions issued through the Service may be filled by, and transferred among, Oriah's licensed U.S. pharmacy partners and delivered to your door.
  • The Service is cash-pay only. You must pay the full cost of the Service and of any prescription, and you will not submit a claim for reimbursement to Medicare, Medicaid, any other federal payor, or any state or private insurer.

12. Concerns and State Medical Boards

The Providers who deliver care through the Service are independent, state-licensed physicians, physician assistants, and nurse practitioners. They are solely responsible for the clinical care you receive, including any weight-loss (GLP-1), longevity/peptide, hormone (TRT), and sexual-health (ED) treatment recommendations and prescriptions. Oriah Wellness LLC ("Oriah") operates the technology platform and provides non-clinical administrative support only; Oriah does not practice medicine, does not provide medical advice, and the provider-patient relationship is with the Providers, not Oriah. Any medications prescribed by a Provider are dispensed by licensed U.S. pharmacy partners, and any laboratory testing is performed by affiliated and/or third-party laboratories.

If you have a concern, question, or complaint about a Provider who has treated you through the Service, you may contact the state medical board in the state where you reside or where the Provider is licensed. Each state medical board maintains a process for verifying a license and for submitting complaints regarding the conduct of licensed medical professionals. Contacting a state medical board is free, and you may do so regardless of whether you have first raised your concern with the Provider or with Oriah.

Verifying a License and Filing a Complaint

Most state medical boards allow you to verify a Provider's license status, view any public disciplinary history, and submit a complaint online. If you are unsure which board has jurisdiction, you may locate your state's board through the Federation of State Medical Boards at www.fsmb.org or by searching for the "[your state] medical board." The Service is available only to adults 18 years of age or older, and these processes apply to you as the adult patient using the Service for yourself.

Special Notice to California Patients

If you reside in California, physicians who treat you through the Service are licensed and regulated by the Medical Board of California. To confirm a physician's license status or to file a complaint, you may contact the Medical Board of California by visiting www.mbc.ca.gov or by calling (800) 633-2322.
